New Patients

Bright Side Eye Care is always welcoming new patients! We have two locations, Oregon and Monona, to better accommodate your needs. You can rely on us for quality services and treatments as we’ve been serving patients since 1942.

 

Want to become a new patient? Give us a call and we'll get you set up for your first visit with us! If you'd like, you can download the new-patient forms below and bring them with you to your first appointment. If you are unable to download the forms, please arrive 10 minutes ahead of your scheduled exam time to complete the paperwork.

This patient portal allows you to access your personal data from anywhere. If you need to know what medication your eye doctor prescribed, when your upcoming appointment is, or to find your latest bill, just log in to find that information.
